When percentage billing fits
A percentage-of-collections arrangement can fit practices with variable revenue or growth because the invoice rises and falls with receipts. Define which receipts count, how refunds and takebacks are handled, and whether patient payments and old A/R are included.
When a flat fee fits
A fixed monthly or per-claim structure can work for stable volumes and clearly bounded responsibilities. It gives the practice a visible unit cost, but exceptions, coding, statements and follow-up may be separate.
Normalize the proposals
Put the same providers, locations, payers, claim types and responsibilities in every request. Then calculate an annual cost under normal, slow and high-volume months before deciding.
